It has come to our attention that a number of bond filers are not submitting the most recent version of the CBP Form 5106 (CBPF 5106). A number of bond filers are either using the previous version (03/99), which expired 09-30-2012 or has generated their own version. The current version of the CBPF 5106 shows 05/13 in the lower right hand corner. This version can be found at http://forms.cbp.gov/pdf/CBP_Form_5106.pdf.

Some of the incorrect versions of the CBPF 5106 show the following at the top of the page:

U.S. Department of Homeland Security
Bureau of Customs and Border Protection

Please note that an accurate version of the CBPF 5106 will show:

Department of Homeland Security
U.S. Customs and Border Protection

Although the current version (05/13) of the CBPF 5106 must be used immediately, the Bond Team will allow a grace period through September 9, 2013 before any further action will be taken. Beginning September 10, 2013, any outdated versions of the CBPF 5106 will be rejected back to the filer.
